On-Call Nurse
Summary of Position: The On Call Nurse is directly responsible to the Team Coordinators for on-call coverage. The On Call Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice.
Position Specifications:
Education: Graduation from an approved School of Nursing.
Licensure: Current registration in the State of Ohio and eligibility to obtain registration in Indiana.
Current registration in the State of Indiana and eligibility to obtain registration in Ohio.
Experience: Hospice nurse experience preferred. Minimum of two (2) years experience in Medical/Surgical, Oncology or community health nursing is preferred.
Special Skills and Requirements:
1. Must have excellent literacy and writing, documentation, communication, and interpersonal abilities.
2. Must have corrected hearing and vision within normal range.
3. Must be able to use tools and equipment required.
4. Shall have a valid driver’s license.
Purchasing/Supply Specialist
Summary of Position: The Purchasing/Supply Specialist is directly responsible to the Director of Property and Risk Management. The Purchasing/Supply Specialist will provide secretarial skills and monitor, order, track, and maintain inventory of supplies.
Position Specifications:
Education: High school diploma or associate’s in administrative services.
Experience: Experience and knowledge of Microsoft Office products.
Minimum of one year secretarial experience required.
Special Skills and Requirements:
1. General secretarial skills with accuracy in typing, filing, and word processing.
2. Excellent communication, documentation, and organizational skills.
3. Excellent customer service skills.
4. Able to follow instructions.
5. Able to work independently without supervision.
6. Must have superb face to face as well as over the phone presence.
7. Shall have a valid driver’s license.
Licensed Practical Nurse
Summary of Position: The Licensed Practical Nurse is committed to the philosophy, goals, and objectives of State of the Heart Hospice. She/he works directly under the supervision of the Team Coordinator.
Position Specifications:
Education: Graduation from an approved School of Practical Nursing.
Licensure: Current registration in the State of Ohio and/or eligibility to obtain registration in Indiana or Ohio.
Experience: Minimum of two years experience as a Licensed Practical Nurse preferred.

Admission Nurse
Two Positions Available
Full Time 9:00 – 5:30 every other Saturday
Full Time 1:00 – 9:30 every other Saturday
Summary of Position: The Admission Nurse is directly responsible to the Admission Coordinator for enrolling patients in the hospice programs and assisting with the referral/intake process. The Admission Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice.
Position Specifications:
Education: Graduation from an approved School of Nursing.
Licensure: Current registration in the State of Ohio and/or eligibility to obtain registration in Indiana or Ohio.
