HOSPICE AIDE

Summary of Position:  The Hospice Aide is a paraprofessional worker, specifically trained to give personal care and assist in care of the seriously ill under the guidance and supervision of a Registered Nurse.  The Hospice Aide is committed to the philosophy, goals and objectives of State of the Heart Hospice.  He/she works directly with the primary nurses of assigned patients and under the supervision of the Team Coordinator.

Education: A High School Diploma is preferred.

Requirements: Completion of State approved 75-hour formal training program consisting of classroom and supervised practical training.  Shall not have a lapse of 24 months of non-compensated aide work. Successful completion of competency evaluation within first four weeks of employment. Pass mandatory written Hospice Aide test within first 6 weeks of employment test. Registered as an aide on the Indiana registry and is currently listed in good standing.

Experience: Previous experience as an aide is desirable.

Special Skills and Requirements:

  1. Must have satisfactory literacy and writing, documentation, communication, and interpersonal abilities.
  2.  Must have corrected hearing and vision within normal range.
  3.  Must be able to use tools and equipment required.
  4.  Shall have a valid driver’s license.

Working ConditionsInvolves both office work and visits to patient homes. Travel may be in inclement weather.  Must be able to lift 25 lbs. of equipment/supplies.  Needs to be able to assist with patient care activities such as ambulation, transfers, and positioning.  Requires varied amounts of sitting, standing, and turning.

Essential Duties:Complete tasks as assigned on the patient’s aide care plan which include:

  1. Provision of hands on personal care.
  2. Performance of simple procedures as an extension of therapy or nursing services.
  3. Assistance in ambulation or exercise.

 

INPATIENT & PALLIATIVE CARE REGISTERED NURSE (Care Center RN)

Summary of Position: The Inpatient and Palliative Care Nurse is responsible for providing primary nursing services to patients and their families in the Inpatient Care Center. The Inpatient and Palliative Care Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice. She/he works directly under the supervision of the In Patient Hospice Director.

Education: Graduation from an approved School of Nursing.

Licensure:

  • Current registration in the State of Ohio and eligibility to obtain registration in Indiana
  • Current registration in the State of Indiana and eligibility to obtain registration in Ohio

Special Skills and Requirements:

  1. Must have excellent literacy and writing, documentation, communication, and interpersonal abilities.
  2. Must have corrected hearing and vision within normal range.
  3. Must be able to use tools and equipment required.
  4. Shall have a valid drivers license.
  5. Demonstrates interest and empathy for the terminally ill and is comfortable with issues of death and dying.

Working Conditions: Involves both office work for charting and direct patient care. Must be able to lift 25 lbs. of equipment/supplies. Needs to be able to assist with patient care activities such as ambulation, transfers, and positioning. Requires varied amounts of sitting, standing, and turning.

Essential Duties:

  1. Provide competent nursing care to assigned patient/families through skilled symptom management, emotional support, appropriate teaching and timely response to patient/family needs.
  2. Implements the complete nursing process for assigned patients and their families

 

PRIMARY REGISTERED NURSE

Summary of Position: The Primary Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice. She/he works directly under the supervision of the Team Coordinator.

Education:Graduation from an approved School of Nursing

Licensure:

  • Current registration in the State of Ohio and eligibility to obtain registration in Indiana
  • Current registration in the State of Indiana and eligibility to obtain registration in Ohio

Experience: Minimum of two (2) years experience in Medical/Surgical, Oncology or community health nursing is preferred

Special Skills and Requirements:

  1. Must have excellent literacy and writing, documentation, communication, and interpersonal abilities.
  2. Must have corrected hearing and vision within normal range.
  3. Must be able to use tools and equipment required.
  4. Shall have a valid driver’s license.

Working Conditions: Involves both office work and travel to visit patients. Travel may be in inclement weather. Must be able to lift 25 lbs. of equipment/supplies. Needs to be able to assist with patient care activities such as ambulation, transfers, and positioning. Requires varies amounts of sitting, standing, and turning.

Essential Duties:

  1. Provide competent nursing care to assigned patient/families through skilled symptom management, emotional support, appropriate teaching and timely response to patient/family needs.
  2. Coordinate IDG planning for assigned patient/families. Participate in IDG analysis of patient/family needs, strengths, and priorities for care and in resulting development, implementation and revision of the IDG Plan of Care.

 

SECOND SHIFT RN

Summary of Position: The 2nd Shift Nurse is directly responsible to the Team Manager for 2nd shift coverage. The 2nd Shift Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice.

Education: Graduation from an approved School of Nursing.

Licensure:

  • Current registration in the State of Ohio and eligibility to obtain registration in Indiana.
  • Current registration in the State of Indiana and eligibility to obtain registration in Ohio.

Experience: Hospice Nurse experience preferred. Minimum of two (2) years experience in Medical/Surgical, Oncology or community health nursing is preferred.

  2. Provide patient/ family services according to established Plan of Care and signed physical orders. Consult patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in Plan of Care.

 LPN

Summary of Position: The Licensed Practical Nurse is committed to the philosophy, goals, and objectives of State of the Heart Hospice. She/he works directly under the supervision of the Team Manager.

Education: Graduation from an approved School of Practical Nursing

Licensure:

  • Current registration in the State of Ohio and eligibility to obtain registration in Indiana
  • Current registration in the State of Indiana and eligibility to obtain registration in Ohio

Experience: Minimum of two (2) years experience as a Licensed Practical Nurse preferred.

  1. Provide competent nursing care, as appropriate to level of practice, to assigned patient/families through skilled symptom management, emotional support to patient/families and timely response to patient/family needs.
  2. Participate as requested in IDG analysis of patient/family needs, strengths, priorities for care and in implementation and revision of the IDG Plan of Care.
  3. Contribute to assigned team to provide not only routine patient care but also continuous, and crisis care as needed.
  4. Take part of “end of life” care as a team member or as needed to help other teams

EXTENDED HOURS RN (On-Call Nurse)

Summary of Position: The On Call Nurse is directly responsible to the Team Coordinators for on-call coverage. The On Call Nurse is committed to the philosophy, goals and objectives of State of the Heart Hospice.

Education: Graduation from an approved School of Nursing

Licensure:

  • Current registration in the State of Ohio and eligibility to obtain registration in Indiana.
  • Current registration in the State of Indiana and eligibility to obtain registration in Ohio.
