POSITION: ON-CALL NURSE

Position Specifications:
Education: Graduation from an approved School of Nursing.

Licensure: Current registration in the State of Ohio and eligibility to obtain registration in Indiana. Current registration in the State of Indiana and eligibility to obtain registration in Ohio.

Experience: Hospice nurse experience preferred. Minimum of two (2) years experience in Medical/Surgical, Oncology or community health nursing is preferred.

Special Skills and Requirements:

  •  Must have excellent literacy and writing, documentation, communication, and interpersonal abilities.
  • Must have corrected hearing and vision within normal range.
  •  Must be able to use tools and equipment required.
  •  Shall have a valid driver’s license.

Working Conditions: Involves both office work and traveling to patient homes. Travel may be in inclement weather. Must be able to lift 25 lbs. of equipment/supplies. Needs to be able to assist with patient care activities such as ambulation, transfers, and positioning. Requires varied amounts of sitting, standing, and turning.

Essential Functions:

  •  Provide competent nursing care during on-call periods through skilled symptom management, emotional support, appropriate teaching and timely response to patient/family needs.
  • Provide patient/family services according to established Plan of Care and signed physician orders. Consult patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in Plan of Care.

Responsibilities of Position:

  •  Provide competent nursing care during assigned on-call periods through skilled symptom management, emotional support, appropriate teaching and timely response to patient/family needs.
  •  Provide patient/family services according to established Plan of Care and signed physician orders. Consult patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in Plan of Care.

 

POSITION: MEDICAL SECRETARY

Position Specifications:

  • Education: High School Diploma required. Associate Degree in Medical Records (A.R.T) or equivalent preferred.
  • Experience: Two years experience in a Medical Records department or equivalent medical setting preferred.

Special Skills and Requirements:

  1. General secretarial skills with accuracy in typing, filing, and word processing.
  2. Good communication, documentation, and organizational skills.
  3. Commitment to confidentiality of records.
  4. Shall have a valid driver’s license.

Working Conditions: Involves computer (word processing, database, etc.) telephoning/answering, and filing. May travel to other sites as requested. Travel could be in inclement weather. Must also be able to lift 10 lbs. of supplies/papers. Requires approximately 6 hours of sitting, and 2 hours of standing, twisting, turning, bending, lifting and walking.

Essential Functions:

  1. Initiate and maintain a current and complete Medical Record for each patient. Ensure current Certification and Re-Certification forms are completed for each patient. Maintain current filing of all clinical records in Medical Records.
  2. Maintain On-Call Logs.
  3. Assist the Finance Department with auditing Medical Records prior to billing as requested.
  4. Endure Plan of Care and/or orders are signed and returned by physicians in the time frames established in the policy.

Responsibilities of Position:

  1. Use correct CPT and ICD codes by maintaining coding skills.
  2. Compile, type, edit, and distribute clinical staff correspondence, reports and other information. This includes IDG and On Call information.
  3. Notify appropriate staff of and changes to patient’s status.
  4. Create, revise, and maintain clinical forms as directed by Department Supervisors Team. Order forms as needed.
  5. Answer telephones as needed to assist administrative staff. Act as a back up for closing office at the end of day when needed.
  6. Keep chart room neat, clean and orderly.
  7. Perform other duties as assigned.
