Full Time Hospice Aide in Jay County

Description:
The Hospice Aide is a paraprofessional worker, specifically trained to give personal care and assist in care of the seriously ill under the guidance and supervision of the registered nurse. The Hospice Aide is committed to the philosophy, goals and objectives of State of the Heart Hospice. He/she works directly with the primary nurses of assigned patients and under the supervision of the Team Coordinator.
Education:
A High School Diploma is preferred.
Requirements Include:
Completion of a 75 hour formal training program. Prefer completion prior to employment.
Demonstration of competency on each skill's checklist mandatory within first 4 weeks of employment.
Mandatory to pass written Home Health Aide test within first 6 weeks of employment or as test available.
Hospice Aides working in the state of Indiana will be entered onto the Home Health Aide Registry and will be reviewed each year of employment.
Experience:
Previous experience as an aide is desirable.
Full Time Social Worker

Description:
The Medical Social Worker is responsible for providing consultation to the Interdisciplinary Team and direct service to the patient/family. He/she is committed to the philosophy, goals and objectives of State of the Heart Hospice.
Education:
Masters of Social Work Degree from a social work curriculum accredited by the Council on Social Work Education.
Licensure:
Current registration in the State of Indiana and eligible to obtain registration in Ohio.
Requirements Include:
Good interpersonal skills, knowledge of family dynamics and counseling skills are essential.
Ability to communicate effectively both verbally and in writing.
Shall have a valid driver’s license.
Experience:
Minimum of two (2) years experience in health care setting is preferred.
