Patients and families often ask how they can thank their State of the Heart caregivers. You may choose to honor them by making a financial donation through our Shining Star program. The Shining Star program allows you to honor your State of the Heart physician, nurse, hospice aide, social worker, chaplain, music therapist, bereavement specialist, volunteer or other individual who made a difference in your care.

Just complete the Shining Star card with the name of your special caregiver or team and enclose your donation. Then mail it to:

Your Shining Star caregiver will receive an award and a card informing them of your thoughtful gift. All donations will support the patients and families we serve.
